You will need to provide us with your logo and shop information in order for us to create your customized folder. Please fill out the information below and then you will be taken to a page where you can upload your image files. (Please note, the files must be in an image format, such as .tif, .jpg, .eps, .gif or .bmp. Do not give us Word docs, Publisher files or other application files, these will be rejected.) Fields below with an * are required. If you don't have an electronic version of your logo available, you can mail us a hard copy and we will scan it. Do not fax us your logo, we cannot scan a workable image from a fax. If you don't have any kind of logo we will be happy to set your shop name in a handsome text format.
